Milwee Middle School

As the end of the school year approaches, we would like to remind you that the
Florida Department of Health and the Department of Education requires all students
entering 7th grade to have a Tdap booster (adult strength tetanus, diphtheria and
pertussis) vaccine or have a related exemption form on file at the school.

If you don’t have a pediatrician, don’t have insurance, or if your insurance does not
cover the cost of vaccines, and your child is starting 7th grade, the Florida Department
of Health in Seminole County (DOH-Seminole) in collaboration with Seminole
County Public Schools Health Services will be hosting special Tdap Vaccination
Clinics at the following schools:

1. Sanford Middle School – March 12, 2025 (4:00pm- 7:00pm)
2. Teague Middle School – March 26, 2025 (4:00pm- 7:00pm)
3. Milwee Middle School – April 2, 2025 (4:00pm- 7:00pm)
4. Tuskawilla Middle School – April 9, 2025 (4:00pm- 7:00pm)
5. Millennium Middle School – April 16, 2025 (4:00pm- 7:00pm)

To pre-register, please visit the DOH-Seminole immunization portal at FL
Immunization Screening Portal | Immunization Form (floridahealth.gov) to complete
the forms ahead of time and expedite your visit.

Please follow these steps:

• After completing the top portion of the intake form on the portal, under “Reason for Visit” please select “Tdap Clinic.”
• Type your preferred school Tdap clinic site under “Other Reason for Visit” to let us know which Tdap clinic site you plan to attend.
• The portal will guide you through the forms that you will need to complete.

A staff member from the DOH-Seminole will review your submission and will
contact you to verify and confirm your information.

Parent must be present for Tdap administration.

This service will be provided at no cost to the parents.
