Registration

If your student would like to register for tutorial, please fill out this form and return it to his/her tutorial teacher (see Tutorial Schedule below). Once the form is submitted, your child can begin attending. At this time transportation is NOT provided. *This form only needs to be completed one time per school year, not every week.
